If your skin is dry, tight, flaky or suddenly reacting to products it used to tolerate, the problem is often not what you're missing on the surface — it's the barrier underneath. Ceramides are the lipids that hold that barrier together, and understanding them explains why some "simple" moisturisers quietly outperform far flashier serums.

What ceramides are

Ceramides are a family of lipid (fat) molecules. Structurally, each is a sphingosine backbone joined to a fatty acid by an amide bond — a long, water-repelling molecule purpose-built to sit between skin cells and keep water in. They are not an exotic additive: ceramides make up roughly half of the lipids in the outermost layer of your skin, and your body produces them naturally.

The bricks-and-mortar barrier

The classic way to picture the stratum corneum — the skin's outermost layer — is as a brick wall. The skin cells (corneocytes) are the bricks, and a lipid matrix of ceramides, cholesterol and free fatty acids is the mortar holding them together. When that mortar is intact and well organised, water stays in the skin and irritants, allergens and microbes stay out.

Most "sensitive skin" is really barrier-damaged skin. Repair the mortar and the sensitivity, redness and tightness usually follow it down.

Why they run low

Ceramide levels are not fixed. They fall with age, which is part of why mature skin tends to be drier. They drop in cold, dry weather. And they are stripped by the things we do to our skin: harsh or frequent cleansing, over-exfoliation, and strong actives used too enthusiastically. A depleted barrier shows up as tightness, flaking, stinging when products are applied, dullness, and breakouts — the signs of water escaping (transepidermal water loss) faster than the skin can hold it.

Do topical ceramides work?

Yes — with a sensible caveat about formulation. Applying ceramides helps replenish the barrier's missing lipids, and ceramide-containing moisturisers have good evidence for improving dryness and supporting compromised or eczema-prone skin. The nuance is that ceramides work best alongside cholesterol and fatty acids in a balanced blend, mirroring the skin's own lipid matrix, rather than as a lone "ceramide" headline ingredient. A well-built barrier cream beats a single lipid in isolation.

How to use them

Ceramides live in moisturisers and barrier creams rather than thin serums. Apply after any water-based actives, ideally while skin is still slightly damp, and they pair beautifully with humectants such as hyaluronic acid, glycerin and urea (which draw in water) and with occlusives (which seal it). Their best role of all may be as a support act: using a ceramide moisturiser alongside retinoids or exfoliating acids cushions the irritation those actives can cause and lets you tolerate them for longer.

Who they suit

Almost everyone benefits, but ceramides are essential for dry, mature, sensitive or eczema-prone skin, for anyone who has over-exfoliated or over-used actives, and for getting through winter. Even oily skin has a barrier to protect — ceramides are not heavy or pore-clogging by nature.

The verdict

Ceramides are not a trendy hero ingredient so much as foundational maintenance — the mortar that keeps everything else working. They will not resurface or brighten, but a healthy, ceramide-rich barrier is what lets the rest of your routine perform without leaving skin dry and reactive. If your skin is unhappy, start here.
