Azelaic acid is one of dermatology's quiet workhorses. It rarely trends the way retinol or vitamin C does, yet few actives can claim to treat acne, calm rosacea and fade pigmentation all at once — while being gentle enough to use during pregnancy. If you have combination skin that breaks out, flushes easily, or scars darkly after a spot, this unassuming molecule deserves your attention.
What it is
Azelaic acid is a naturally occurring dicarboxylic acid — a simple nine-carbon chain with a carboxylic acid group at each end. It is found in grains such as rye, wheat and barley, and is also produced on our own skin by Malassezia furfur, a commensal yeast. That last fact is more than trivia: the patches of lightened skin seen in the yeast-driven condition pityriasis versicolor first hinted that azelaic acid could interfere with pigment production, which set researchers on the trail of its skin-lightening properties.
How it works — on several fronts at once
Part of azelaic acid's appeal is that it does not rely on a single mechanism. Researchers have identified at least four distinct actions, which is why it suits such different complaints.
It fades pigmentation selectively. Azelaic acid is a reversible inhibitor of tyrosinase, the enzyme that drives melanin production. Crucially, it appears to act preferentially on hyperactive, abnormal melanocytes rather than on normal ones — so it tends to lift dark marks without bleaching the surrounding skin. This selectivity is a meaningful safety advantage over some other lightening agents.
It is antimicrobial. It is bactericidal against Cutibacterium acnes (the bacterium implicated in acne) as well as Staphylococcus species, and — notably — no resistant strains of C. acnes have been identified, in contrast to the resistance problems seen with topical antibiotics.
It calms inflammation. Azelaic acid scavenges reactive oxygen species and dials down several drivers of inflammatory skin disease, including kallikrein-5, the cathelicidin LL-37 and toll-like receptor 2 (TLR-2) — the same overactive pathways now understood to underlie rosacea.
It normalises the pore lining. It has mild comedolytic and anti-keratinising effects, helping to keep follicles from becoming blocked in the first place.
What the evidence supports
The clinical record here is unusually solid for a skincare active. A 2023 systematic review in the Journal of Cosmetic Dermatology identified 43 randomised controlled trials. For rosacea, the 15% gel significantly improved redness, inflammatory lesion counts and overall clearance versus vehicle, and was found at least as effective as — in places more effective than — topical metronidazole. For acne, 15–20% formulations produced reductions in inflammatory and non-inflammatory lesions comparable to topical retinoids such as adapalene, with better tolerability in some studies. For melasma and post-inflammatory hyperpigmentation, the 20% cream has shown efficacy similar to hydroquinone with a more favourable safety profile.
For deeper skin tones especially — where a single spot can leave a dark mark that outlasts the spot by months — azelaic acid's ability to treat the acne and the pigmentation it leaves behind is a genuine two-in-one.
Strengths and concentrations
Prescription azelaic acid comes as a 15% gel (licensed for papulopustular rosacea) and a 20% cream (for acne); in the UK these are sold under names such as Finacea and Skinoren. Over-the-counter cosmetic products typically use around 10%, often as a suspension. The higher prescription strengths have the bulk of the trial evidence behind them; the lower over-the-counter percentages are gentler and useful for maintenance, but less rigorously studied. It is also worth knowing that azelaic acid is poorly soluble and does not absorb readily, so the vehicle matters — a well-formulated gel can deliver far more active into the skin than a basic cream.
How to use it
Apply a thin layer once or twice daily to clean, dry skin. A mild, transient tingling or warmth on application is common and usually settles within a couple of weeks; genuine irritation is uncommon. Results are gradual — think two to three months rather than two to three weeks — so consistency matters more than intensity. It layers comfortably with niacinamide and gentle hydrators, and as ever, daily SPF is non-negotiable when treating any form of pigmentation.
Its standout practical advantage: azelaic acid is classified as pregnancy category B and is considered safe in pregnancy and breastfeeding, making it one of the very few effective options for acne or melasma when retinoids and hydroquinone are off the table.
The verdict
Azelaic acid is a rare combination of versatile, well-evidenced and well-tolerated. It will not deliver the dramatic resurfacing of a high-strength retinoid, and the over-the-counter strengths ask for patience. But for combination and sensitive skin, for rosacea-prone redness, and for the stubborn dark marks left by past breakouts, it is one of the smartest and safest actives you can add to a routine.
